The Consequences of Untreated Hearing Loss: Why You Shouldn’t Wait to Seek Help for a Hearing Difficulty

After over 30 years as an audiologist, I have heard a lot of reasons from a lot of patients as to why they had not addressed their hearing difficulties sooner.

And of course, I always explain to them that ignoring a hearing difficulty won’t make it go away, and that there are consequences for not treating a hearing loss.

Audiologist & Ear Doctor, Dr. Crystal Chalmers, Chico, CaliforniaNow there is even more evidence – gleaned from scientific studies by some of the top researchers in the world –  to support what audiologists like myself and many of my colleagues with Audigy Group have been saying all along: don’t wait to get tested and – if the tests do indeed show a hearing loss – discuss treatment options with your hearing care professional.

These studies clearly show that hearing loss doesn’t just affect one’s ability to hear the TV or communicate with a spouse, friends, or co-workers.  There can be serious implications for brain atrophy and cognitive function.

Study Results Released at Audiology Conference

A number of these studies were presented and discussed at the American Academy of Audiology’s “Audiology NOW! 2012” annual international conference which was held March 28 thru March 31 in Boston, Massachusetts.   I attended this conference and took in several of these workshops, notably one titled “Issues in Cognition, Audition, and Amplification: A Panel Discussion”.

This extremely informative workshop was presented by a panel comprised of six highly respected researchers from the USA, Canada, and northern Europe.

The presenters and their topics were:

● Larry E. Hulmes, Ph.D., Indiana University “Higher Level Processing Abilities”

● Ulrike Lemke, Ph.D., Senior Researcher, Phonak International, Zurich, Switzerland “The Cognitive Part of Successful Speech Recognition”

●  Brent Edwards, Ph.D., Starkey Research Laboratories, Eden Prairie, MN “How Hearing Aid Technology Can Affect Cognitive Function”

● Kathy Pichora-Fuller, Ph.D., University of Toronto, Canada “Can Hearing Aids Accelerate Listening and Speech Understanding?”

● Thomas Lunner, Ph.D., Eriksholm Research Center, Oticon International, Denmark, and Department of Behavioral Sciences and Learning, Linkoping, Sweden “Memory Systems in Relation to Hearing Aid Use”

● Frank Lin, M.D., Ph.D., Johns Hopkins Medical Institutions, Baltimore, MD “Hearing Loss and Dementia”

Collectively these presenters showed how brain function and hearing loss are intertwined and that when left untreated hearing loss clearly affects cognitive function.  Of special interest were findings on how the use of hearing aid technology could produce marked improvements in those who had had cognitive decline caused by untreated hearing loss, much the same as physical therapy can be used to restore deteriorated muscle mass.

University Study Links Brain Atrophy with Hearing Loss

A recent study by researchers at the Perelman School of Medicine at the University of Pennsylvania – the results of which were released in August of last year – show that declines in hearing ability may accelerate gray matter atrophy in auditory areas of the brain.  This shrinkage of the brain in areas important for hearing comprehension can cause an increase in the effort needed by the affected person to successfully comprehend speech.  This need for increased effort can be particularly troublesome for older adults as the amount of energy used for “hearing” can impact other areas in the affected person’s daily life.

The study found that people with hearing loss showed less brain activity on functional MRI scans when listening to complex sentences.  Poorer hearers also had less gray matter in the auditory cortex, suggesting that areas of the brain related to auditory processing may show accelerated atrophy when hearing ability declines.  In general, the study showed that loss of hearing sensitivity “has cascading consequences for the neural processes supporting both perception and cognition.”

“As hearing ability declines with age, interventions such as hearing aids should be considered not only to improve hearing but to preserve the brain,” said lead author Jonathan Peelle, Ph. D., research associate in the Department of Neurology.   The research for this study can be found in The Journal of Neuroscience and was funded by the National Institutes of Health.

Consumer Protection Laws & Regulations for Hearing Aid Purchases

Here in California – as in many states – purchasers of hearing aids are afforded protections under state laws and regulations administered by the Department of Consumer Affairs (DCA) through the Speech-Language and Audiology and Hearing Aid Dispenser Board (SLPAHADB).

The SLPAHADB was formed on January 1, 2010 by combining the Speech Language Pathology and Audiology Board with the Hearing Aid Dispensers Board.  Previous laws and regulations remain virtually unchanged except that audiologists are no longer required to hold dual licensure for practicing audiology and dispensing hearing aids.

Audiologist & Ear Doctor, Dr. Crystal Chalmers, Chico, CaliforniaThis board’s mission is ensuring the competency and fair practices of hearing aid dispensers.  It does so by providing licensure through testing, ethical guidelines, boundaries for advertising practices, and acts as a vehicle for consumer complaints, ultimately providing enforcement and disciplinary functions.

While political discussion in the media about government intrusion in our lives is a hotbed topic, consumer protection laws are generally an accepted interaction between the citizenry and government.  Who wants to buy poisonous food, cars that don’t run, or housing that collapses in the slightest breeze, to name just a few examples?

Why should it be any different with hearing aids?   Purchasers have a right to expect fair and honest treatment as well as professional expertise from those they seek help.  It could be argued that hearing aid purchasers – many of whom are our senior citizens – should receive an added degree of protection as many of our laws are designed to protect us as we age beyond our more active years, and thereby tend to rely on the care and guidance of those who are younger.

While all the laws and regulations are too lengthy to list here, these are several of the most important.  For the full listings, visit the SLPAHADB website at www.speechandhearing.ca.gov

  • 30 Day Warranty  – The “Song-Beverly Consumer Warranty Act” provides a 30 day warranty on all new and used hearing aids.  If the hearing aid is not specifically fit for the buyer’s particular needs, the device may be returned to the dispenser within 30 days of the of the date of the actual receipt by the buyer or completion of fitting by the seller, whichever occurs later.  If the buyer returns the device the seller must either adjust or replace the device or promptly refund the total amount paid.

It should be noted that while many dispensers advertise things like “30 day risk-free guarantee” they are merely staying within the state mandated minimum trial period of 30 days.  My professional opinion is that for many purchasers, 30 days is too short as it does not allow enough time for the brain to get used to hearing again.  In my practice, I offer a 75 day trial period in order to make certain my patients have all the time they need to be successful.

  • Mail Order & Internet Sales  – California law provides that mail order/Internet hearing aids may only be purchased through a dispenser licensed in California.  The law also states that that when hearing aids are purchased by mail order/Internet, there must be no fitting, selection, or adaptation of the instrument and that the seller must not give any advice with respect to the taking of an ear impression(s). Anyone contemplating this avenue should be aware of potential risks as to work effectively, hearing aids must fit correctly.  If a sale doesn’t involve personal contact between the dispenser and buyer, it is difficult to assume that proper fitting and follow-up care could occur.
  • In-Home Contracts & Cancellation Rights – If you sign a hearing aid purchase contract in your home, federal and state laws allow you to cancel it for any reason by midnight of the third business day after you signed the contract.
  • Advertising Guidelines –  Did you know that in California, hearing aid dispensers are not licensed to perform diagnostic hearing evaluations (only audiologists are licensed to do so) and are therefore not allowed to charge a fee for “testing”?  That is why they advertise “free” tests, BUT they must state that the “test” is “to determine if you could be helped by a hearing aid.”

No one selling hearing aids in California can refer to themselves as a “specialist” without including the title “hearing aid dispenser”, nor can anyone refer to certification by putting a bunch of letters after their name.  For example, this listing: “John Doe, NB-HIS” is unlawful.  The correct listing should be:  “John Doe, Hearing Aid Dispenser, Lic. No. XXXX, NB-HIS, Certified by the National Board of Certification in Hearing Instrument Sciences”

Have you ever received one of those “rebate coupons” that resemble checks as part of a direct mail solicitation?  It is a violation to send those to anyone living in California.

And finally, no California licensed hearing aid dispenser can use the terms “doctor” or “physician” or “clinic” or “audiologist” or any derivation thereof, except as authorized by law.

These are just a few of the consumer protections for purchasing hearing aids.   I urge you to be good consumers by getting the facts and educating yourself about where you stand and whom it is that you are considering doing business with.  You’ve worked hard all your life and deserve to be treated fairly, professionally, and with what is in your best interest  …. Because hearing is a wonderful gift!

You Heard Right. Patients are Happy with Hearing Aids!

Why don’t people use hearing aids? If asked, many people are hesitant based on the fact that they have a friend, who has an aunt, whose husband spent a great amount of money on hearing aids that he ended up hating. There are indeed many “hearing aid horror stories” circulating the popular bridge clubs and local fishing holes. “They were uncomfortable, they made everything louder, they whistled, and of course, they costs so darn much.” There was a time that these were valid complaints from hearing aid users. However, we are happy to report that those times are over.

In 2011, it was reported that 74% of hearing aid users are satisfied with their hearing aids, a 55% increase from 2000. Furthermore, satisfaction with hearing aids that are less than one year old has increased to 81%. And believe it or not, 82% of hearing aid users would recommend hearing aids to a friend. The patient population of hearing aid users has not changed much in the past ten years. So what has changed to account for this increase in satisfaction? (MarkeTrak VIII, 2010)

There has been much advancement in the past ten years resulting in greater patient experience and satisfaction with their hearing aids. The hearing aid technology itself has exponentially advanced. Equally important, the Audiologists providing hearing care have also grown in their knowledge and patient care.

As with every other modern technology, be it cell phones or computers, new hearing aid technologies are also rapidly emerging. These advancements have focused on increasing audibility of a speaker in a difficult listening situation, such as background noise. Hearing aids are now automatically processing when “noise” or unwanted sound is present and reducing it. Likewise it is also processing when “speech” or a desired listening signal is present and increasing it. Most of the hearing aids dispensed today have some amount of automatic adjustments making it so patients don’t have to adjust the hearing aids themselves. With new hearing aids performing countless adjustments per millisecond, it’s no wonder patients are hearing better in difficult environments.

Another emerging technology in hearing aids is Bluetooth. In its simplest form, Bluetooth technology allows patients to directly receive audio input through their hearing aids. For example, if a person is using a cellphone with Bluetooth compatible hearing aids, the signal from the phone is routed through the hearing aid rather than through the phone allowing the patient to “hear” the phone directly through the hearing aids. This technology is also available for the television, landline phones, or iPods. It can even help hear a spouse in a difficult environment, such as a restaurant or the car, by using a small lapel microphone. These advancements have added another level of satisfaction for hearing aid users and their families.

There has also been much advancement in the styles of hearing aids offered today. Gone are the days of the Grandma Monster Behind-the-ear or In-the-ear hearing aids. New digital hearing aids, even the behind the ear style, have become more streamlined and cosmetically appealing. In fact, there are now some hearing aids that fit completely down the ear canal and are 100% invisible. Now even the most cosmetically concerned patients can experience the benefits of better hearing without the embarrassment of others knowing they have a hearing aid.

More important than the technological advancements in the hearing aids or design of the hearing aids, patients are more satisfied with the care they are receiving from their Audiologists. Many Audiologists have now obtained their Doctor of Audiology, or Au.D., which has their increased the education and training. In addition to that, some Audiologists have obtained certification, such as AudigyCertification, in addition to the entry level requirements as a commitment to premier patient care. This certification assures patients that they are receiving expert advice by the highest trained and credentialed Doctors of Audiology. It also helps patients know that they will be recommended appropriate technology based on their unique needs and listening environments, and not be recommended technology that is the latest hearing fad or on a special one day sale. Additionally, by instituting patient-centered care, the Audiologist, patient and significant family members work closely with one another. The patient is routinely seen for aural rehabilitation, which involves retraining the brain to hear again, through the trial period and beyond to make sure that they are experiencing better quality of life through better hearing.

Baby Boomers: The Next Generation to have Hearing Loss

For many of you between the ages of 46 to 64, your time is coming.

Time, that is, to experience what so many of your parents and grandparents came to learn of fist hand: hearing difficulties caused by exposure to excess loud noise.

The “Baby Boomer” generation – those Americans born between 1946 and 1960 – accounts for some 76 million of us in the Untied States today and at least 15 percent of Boomers already have hearing loss.

This is a far cry from the previous 2 generations, who typically did not show symptoms of hearing loss until they were in their 70’s and 80’s.  Indeed, when I was an audiology student at Minot State University earning my Master’s of Science degree I was taught that only men in their 70’s and 80’s got hearing loss.

What has changed?  Our world is now much, much louder.

An Unprecedented Century

Prior to the Industrial Revolution of the 1890’s, relatively few Americans were exposed to loud noise.  At the turn of the 20th Century, much of our population inhabited rural areas with males working in non-mechanized agriculture and females involved in homemaking chores, which they performed by hand.  There were no vacuum cleaners or food processors.

However, with unprecedented technology, mechanization, and involvement in two World Wars, the US population got its first dose of exposure to excessive levels of loud noise.

Still, the field of audiology – born of the technological research in sonar for the Navy in WWII  — was in its infancy in the 1950’s and was relying on the only set of data on hearing loss available till then, which was testing conducted at the 1939 World’s  Fair in San Francisco.

But a funny thing happened.  That data was proven wrong when, in the mid 1980’s women in their 60’s started showing up in audiologist’s offices across the country with identical hearing loss of their male counterparts.

Audiologist & Ear Doctor, Dr. Crystal Chalmers, Chico, CaliforniaWho were these women and why, for the first time in recorded history, were they suddenly experiencing hearing loss?  She was, in fact, the “Rosie the Riveters” of WWII: those young American women who, with all the men serving in the armed forces, went to work in the factories and shipyards providing the “Arsenal of Democracy” with much needed war effort labor.  These women built the Liberty ships, Sherman tanks, and B-17 bombers used to defeat the Axis Powers …. And got themselves a hefty dose of impaired hearing in the process!

Post War Boom

After victory in WWII Americans got busy in the greatest economic surge in human history … and had babies.  Lots of babies.

And as those babies grew into young adults in the 60’s and 70’s a flourishing American economy provided them with all sorts of loud recreational activities.  From rock concerts to ski boats to hot rods to stereo systems, Americans got a steady diet of loud noise long before subsequent research showed the direct correlation between the noise and hearing loss.

Early Hearing Aids: Low-Tech = Bad Reputation

Parents of the Boomers had lousy timing as hearing aids back then were pretty awful.  These devices only made sounds louder for the wearer.  While a few people benefited from this simplistic approach to solving hearing difficulties, most did not and hearing aids got a deserved bad reputation.

So bad in fact that after graduating from school, I steered my career towards diagnostics, performing testing of the hearing system for the determination of medical problems such as the location and size of brain tumors and loss of balance problems.

The Digital Breakthrough

Everything about hearing aids changed in the late 1990’s with the breakthrough of digital hearing technology.  In essence, digital hearing aids are miniature computers that dramatically increase the amount of sound processes possible (and available to the wearer) as opposed to the old linear and analog products.  And digital products keep getting better, with several major advancements taking place during the last decade with the last two years seeing some terrific gains.

Today’s digital products are so advanced we don’t even call them hearing “aids” anymore.  Instead we prefer to call these amazing products “hearing technology” as they are smaller than ever with superb sound quality.  Top-of-the-line models have features that Boomers need such as “directionality” for enhancing sound coming to from the front, while tuning down sound coming from behind such as someone might experience at a noisy restaurant or party.  Also, Bluetooth capability enhances the lives of 21st century on-the-go active Americans.

Educate Yourself: Get the Facts about Hearing Care

The myths and misperceptions associated with hearing loss and hearing aids of 30 years ago should be put to rest.  The science of Audiology has come a long way as has hearing technology.  No one needs to suffer all the serious communication issues associated with hearing difficulties … if they would only seek help.

SIDEBAR:  More baby boomers showing signs of hearing loss

  • More than 55 million Americans have some degree of hearing loss—approximately one in 5 individuals — and this number is expected to increase further by 2030.  Much of that looming surge is a baby-boomer phenomenon.
  • Among Americans ages 46 to 64, about 15 percent already have hearing problems, according to a survey by the Better Hearing Institute.
  • Two out of three people with hearing loss are below retirement age.
  • Sixty percent of people with hearing loss are male.
  • Only 12 percent of physicians today ask patients if they have any hearing problems.
  • Only one in five people who could benefit from hearing aids currently wear them.

With the start of the New Year, we are reaching an important date here at North State Audiological Services.

Demystifying Audiograms: Taking an audiogram and how to understand the results

If you’re one of the nearly 40 million Americans who may suffer from hearing loss, there’s a good chance you’re in need of an audiogram. The audiogram determines, for each ear, the degree of hearing loss you are experiencing.

Now that you’ve been using the appropriate hearing aids and your brain has been retrained to hear with their assistance, it may be time for another audiogram. The good news is, thanks to your devices and the adaptive capabilities of the human brain, your next test results may place you within the “normal” range of hearing.

As you may recall, an audiogram records the results of a hearing test in a graph of symbols. Each ear will have a table and a graph showing how well one can hear sounds at various frequencies, which are measured in units of cycles per second known as Hertz (Hz). As frequency increases, so does pitch. The degree of loudness or intensity is measured in units called decibels (dB).

The audiogram will measure the lowest volume (or decibel) that you can hear pure tone signals at different frequencies, for your left and right ear. The softest sounds you hear at each pitch, at least 50 percent of the time—known as your hearing threshold—will be classified as your hearing level.

For example, a bass drum is a low frequency-low pitch sound, while a flute or violin is a high frequency-high pitch sound. An audiogram’s graph displays low to high frequencies (low to high pitches) from left to right, much like the keys on a piano; soft sounds appear at the top of the graph, loud sounds at the bottom. A person’s low frequency thresholds are read first, followed by the mid and high thresholds.

The results of an audiogram can be clarified when combined with other tests, such as the ability to recognize or understand words, as well as hearing capability in noisy environments.

The Softest Sounds You Hear

Hearing is measured in decibels (dB), a recognized unit of loudness tied to a scale. The softest sound you can hear is considered your hearing threshold:

  • Normal hearing: 0 to 20 dB (can hear chirping birds)
  • Mild: 21 to 40 dB (can hear clock ticking)
  • Moderate:  41 to 55 dB (hard to hear or understand regular speech)
  • Moderately-severe: 56 to 70 dB (hard to hear telephone ringing)
  • Severe: 71 to 90 dB (can only hear loud sounds such as sirens)
  • Profound: 91+ dB (trouble hearing motorcycles, power tools, etc.)

Remember, the extent of your hearing loss is an important factor when choosing the correct hearing aid.

The Noise Thermometer*

The Sight & Hearing Association, a nonprofit organization that helps to identify preventable loss of vision and hearing in children, created what it refers to as The Noise Thermometer, a chart used to associate risk of hearing damage to decibel (dB) level:

85 dB: Beginning of OSHA regulations

90 dB: Lawn mower, truck traffic; hearing damage in 8 hours

95 dB: Motorcycle, power saw; hearing damage in 4 hours

100 dB: Chainsaw, stereo headphones; hearing damage in 2 hours

105 dB: Jackhammer, helicopter; hearing damage in 1 hour

110 dB: Snowmobile, driver’s seat; hearing damage in 30 minutes

115 dB: Stadium football game; hearing damage in 15 minutes

120 dB: Rock concert; hearing damage in 7.5 minutes

125 dB: Firecracker; pain threshold

140 dB: Jet engine at take-off; immediate danger to hearing

WHAT DOES DIGITAL MEAN?

Many times patients come to my office and ask about what do those “digital” hearing aids mean to me?  To answer this we have to look back and understand what hearing aids use to be.  Hearing aids generally have three classical definitions for the circuitry inside:  Analog, Programmable, and Digital.

Analog hearing aids are types of circuitry which are set by the manufacturer and give little adjustment to the Audiologist to control.  Analog hearing aids were used for many decades until around 2000-2008 where the manufacturers stopped requesting and making parts for these devices.  Because of the lack of parts available for these devices, it is essentially not repairable and needs to be replaced.

Programmable hearing aids were the beginning of the revolution to allow the Audiologist to change the settings of the aid to better suit his/her patient’s lifestyle more individually.  These were used in the 1990’s primarily and now are virtually unavailable for any repair issues because these parts are also not made anymore.

Digital devices began and started a few times in the 1970-1980’s but began in earnest in the late 1990’s.  Now all devices are digital.  But do not be fooled by sales people telling you that all digital hearing aids are the same because they are not.  Digital devices could have as few as 10 individually adjustments to around 100 separate adjustments that only your AudigyCertified Audiologist can effectively handle.  If, as one of my favorite patients asked me, the birds on the golf course were too loud for you, it can be adjusted effectively and easily.  Many patients have not realized that their devices could be modified to accommodate their specific needs according to their hearing loss and listening lifestyle.

“So can digital hearing aids take away all the sounds I do not want to hear?” This is a common question I hear but the answer is yes and no.  We all hear sounds we do not wish to hear, such as a yelling child in a restaurant, but we all still have to deal with this problem.  What can be fixed is the comfort of that noise to make it not exceed your ability to tolerate that sound (or hurt your ear), unless everyone around you is holding his or her ears as well.

Every year the manufacturers are creating new devices to accommodate the difficult lifestyles our patients see to handle the noise, whistling and other unwanted issues and putting them literally in the rearview mirror of hearing history.  Keep asking an AudigyCertified Audiologist what new technology might be available for you or your loved one.

Hearing Care, the Internet, and “Big Box” Retailers

A full decade into the 21st Century, it would be rare to find an American who has not been to a modern shopping mall or used the Internet.  In fact many of us visit “Big Box” anchored shopping malls at least occasionally, and the number of Americans who regularly use the Internet far exceeds the number who do not.  Even amongst senior citizens — the final demographic to embrace this technology — the gap between Internet users and non-users continues to narrow.

Without doubt, the Internet is a fabulous tool, providing us with a wealth of easily obtainable information in mere seconds.  And the amount of relatively low-priced goods and services that “Big Box” and chain retailers supply can certainly help many families stretch their monthly budget.

But are these two venues the be-all-and-end-all for every consumer scenario?

The short answer is, “Absolutely not!” … especially when it comes to professional hearing care technology and follow-up service.  The Internet outlets as well as “Big Box” and retail chain stores simply cannot provide the level of patient care and service that a full-service, AudigyCertified™ hearing care practice must provide to its patients.

These outlets may tout “low prices” but remember that “price” is only an issue in the absence of value.  And when it comes to hearing care services and technology, value is clearly lacking from these types of retailers.

Do research.  Verify credentials as well as state and federal consumer protection laws.  A good website should educate you.

Did you know hearing loss…

Is not being adequately identified by healthcare professionals? Only 38 percent of adults ages 70 years and older and only 29 percent of adults ages 20 to 69 have had their hearing tested within the last 5 years!

Do you or someone you love have a hearing loss? If so, you’re not alone. Almost 20% of adults in the United States will develop hearing loss during the course of their lives, and if undiagnosed and untreated, its affects can be devastating. Impaired hearing can have a profound impact on emotional, physical, economic, and social well-being.  People with hearing loss have documented decrease in quality of life, reporting symptoms of depression, dissatisfaction with life, reduced functional health, and social isolation. Statistically, they have lower income levels, and frequently complain of frustration in relationships and various communicative situations, as well as inability to enjoy social situations, and fatigue. Despite the fact that most people with hearing loss can be helped with today’s state of the art amplification, many never seek help, or resist the use hearing aids or other assistive listening technologies.

The causes of hearing loss are varied and resultant impact on auditory capability may range from mild to pronounced. Sometimes the cause or etiology, such as wax build up in the external ear canal or an ear infection, is readily apparent. In other instances, the cause of hearing loss may be more obscure. Decreased hearing is often called “the invisible handicap”. Because hearing impairment (especially loss related to aging) often presents very gradually, those with significant hearing impairment can be unaware of their loss. Family members, significant others, co-workers and friends are frequently the first to notice communication problems related to decreased hearing sensitivity.

Given that 36 million Americans are faced with this “invisible handicap” what kinds of risk factors increase the likelihood that someone will develop hearing loss? Why do some people develop hearing loss and while others do not? Some risk factors are obvious, but others are much more obscure.

Perhaps the most widely recognized risk factor is aging. Presbycusis, or the loss of hearing that gradually occurs as we grow older, is a familiar scenario for those of us with older family members. In fact, about 30-35 percent of adults between the ages of 65 and 75 years have a hearing loss, while an estimated 40-50 percent of people 75 and older have a hearing impairment.

Loss associated with presbycusis is usually greater for high-pitched sounds, resulting in the frequent comment “I can hear people talking, but can’t understand them clearly”. It is most commonly binaural (in both ears), affecting the ears equally. Because this type of loss in hearing sensitivity is so gradual, people who have presbycusis frequently lose their frame of reference for normal loudness of sounds, and often do not realize that their hearing acuity is diminishing.

The resultant relationship dynamic is frequently the subject of jokes – “My husband suffers from ‘selective’ hearing and he’s driving me crazy”! In reality, however, hearing impairment is no laughing matter and can take quite a toll on communication with significant others. Research indicates that for couples where one person has unaddressed hearing loss, the divorce rate is actually significantly higher.

Most people would agree that we now live in a very noisy world. Noise levels for rock concerts, radios, motorcycles, traffic, industrial and lawn equipment, and even our personal listening devices constantly bombard our auditory system. It is not surprising that noise exposure is another one of the most frequently cited risk factors for hearing loss, and now rivals aging as the number one cause of hearing loss in this country.  Noise from occupational, recreational and sporting activities all pose significant hazards to hearing in the United States today. Firearm use is one of the biggest culprits in our area of the country. A single shot from a shotgun, experienced at close range, can permanently damage hearing.  Repeated exposures to loud machinery in the work place or as part of yard maintenance or recreational experience may, over an extended period of time, present a serious risk to hearing. Even something as seemingly benign as blow dryers for styling hair put hearing at risk.

Consider the noise levels for such activities as NASCAR, jet ski and power boat usage, and lawn equipment. The cumulative effects of these noise hazards and many others pose a serious risk for hearing. Noise exposure risk is a time weighted function – in other words, the longer the exposure time, the greater the risk, and lower the loudness level required to cause damage. According to the National Institute on Deafness and Other Communication Disorders (NIDCD), 10 million Americans have already suffered irreversible hearing damage from noise, and 30 million more are exposed to dangerous noise levels each day. A one-time exposure to hazardous noise resultant from gunfire or a rock concert, no matter how brief the time, of 120 dB or more can leave hearing permanently impaired. Prolonged exposure to only 85 dB (which can be generated by a commonly used appliance such as a blow dryer or hand held power tool) can do the same. 

Children and young adults are bombarded with potentially damaging noise exposure, most of which is easy to overlook. Most people intuitively recognize that “boom boxes”  IPODS, and other personal listening devices, if played too loudly, constitute a risk to hearing. However, consider the noise levels inherent to playing in a school band or orchestra. Students engaged in this commonplace activity spend hours practice in noisy environments within the context of their academic activities, yet many of them are not wearing appropriate ear protection, nor are schools effectively addressing this problem. To complicate matters, for reasons that are not fully understood some people are more susceptible to noise exposure than others.  As an example, research indicates that those with blue eyes are more prone to noise exposure than those with darker eye color!

Aging and noise exposure are two rather obvious risk factors for hearing loss, but there are many other factors that are more insidious. Studies suggest that there is a strong genetic component inherent to hearing loss, both for childhood deafness as well as presbycusic loss. If your parent or grandparent had hearing loss, your risk factor for developing a similar impairment may increase. If you had a relative who was hearing impaired from birth, family members of child bearing age need to be mindful of this risk factor, and be particularly vigilant with regard to insuring that the newest members of the family are effectively screened at birth for hearing sensitivity.

Various diseases of the ear certainly pose risk to hearing sensitivity. Ear infection, otosclerosis (a bony growth in the middle ear cavity), Meniere’s Disease, acoustic neuroma (a tumor on the auditory nerve), and a host of other maladies are obvious risk factors for hearing loss. But did you know that hearing loss is about twice as common in adults with diabetes compared to those who do not have the disease, according to a new study funded by the National Institutes of Health (NIH)? Visual deficits have long been associated with diabetes, but hearing loss is an under-recognized complication. Because of the strong correlation between diabetes and hearing loss, The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), recently suggested that people with diabetes should consider having their hearing tested.

Use of common drugs such as antibiotics, aspirin, diuretics and chemotherapy can cause hearing loss. Typically, hearing loss from ototoxic drugs is high frequency, which often results in the hearing impairment less obvious to those it affects. All radiation and chemotherapy patients should insist on baseline hearing assessments before, during, and following their course of treatment to insure that auditory effects of ototoxic interventional strategies are carefully monitored and treated.

Race and gender also play a role in hearing loss risk factor determination. Researchers now know that compared to women, men are five and one half times more likely to have hearing loss. White and Mexican American men have a higher incidence of both high-frequency hearing loss and hearing loss in both ears than blacks, who were 70 percent less likely than white participants to present with hearing loss.

With the dramatic increase in airbag use, motor vehicle accidents (MVA’s) are common culprits for increased risk of hearing loss. In fact, 17% of those involved in MVA’s with airbag deployment will have permanent loss of hearing as a result. Other insidious risk factors for hearing loss include smoking and cardiovascular disease. The incidence of hearing loss is significantly more pronounced among smokers, as well as those with any type of cardiovascular disease.

Many healthcare professionals simply don’t make these associations, and as a result, do not refer patients for hearing evaluation as often as risk factors might dictate. So, the message is be aware, and be proactive in assessing your risk, or that of your loved one, for hearing loss. For a comprehensive audiometric evaluation, see a Board Certified Doctor of Audiology. If you’ve never had a baseline audiogram, it’s certainly in your best interest to do so. Only 38 percent of adults ages 70 years and older and only 29 percent of adults ages 20 to 69 have had their hearing tested within the last 5 years.   If you’re over 65, hearing evaluation each year by a Board Certified Audiologist should become part of your annual medical maintenance program.
